FANC/AFCN Patient Release Card

Release card template

A FANC/AFCN technical decree published on August 3^rd 2020 specifies the information that must appear on the discharge card given to a person who has been administered a radioactive product for radiotherapeutic purposes.

Based on this regulation, the FANC/AFCN has developed model release cards in Dutch, French and English that are available for download: here.

For security reasons, the FANC/AFCN requests that the distribution is limited as much as possible.

Attention ! This “Calculator” does not calculate the time periods that measures have to be taken by the patient towards his partner, the public, pregnant women and children,… (limiting the time of contact, keeping distance,…) as currently indicated in the written instructions for the patient.
The FANC/AFCN has not changed the time periods that have to be indicated in these written instructions. More information is available on the FANC/AFCN website.
